There is a new provider for our FSA Card.  P&A group. As of December 15th P&A will handle all the claims and card use verification forms.  So get it in early if you can.To sign up visit the LIRR employee page, (via link below) and simply follow the instructions with the group number.  It's easy and you'll be all set for next year.

Our BRS national organization, in conjunction with our designated counsel, provide us with informational cards regarding our rights in the event of an on the job injury. Being that we are under the jurisdiction of FELA law, it is imperative that we try to understand just what our rights are. Collecting Facts For a Claim or Grievance
The five plus two W's
 
1. Who was involved?
     (a) Name and position?
     (b) Covered by agreement?
 
2. What happened to make it a claim or grievance?
 
3. Where did it happen?
     (a) Did it happen more than once?
     (b) If so. did it happen in more than one period?
 
4. When did it happen?
     (a) Day of the week?
     (b)Date?
     (c) Time of day?
     (d) During or outside the claimant(s) working hours?
 
5. Why did it happen? (Not why it is a grievance.)
     (a) Caution ! Is this a "fact" or "opinion", or a mixture of both"? (Separate
           the two.)
     (b) If you cannot get enough facts to prove your opinion as to why it
           happened, then what are the facts that have led you to your opinion?

     (a)Who?
     (b) Did they observethe actual occurrence, the effects, or both? If neither,
           be prepared to explain.
